All Supplemental Materials must be received (not post-marked) by the deadline stated in the email you will receive after we have received your admissions application.

All applicants are required to submit the supplemental documents listed below. After completion of the UC General Application please download and retrieve these documents as soon as possible.

  • Music Major Questionnaire – Required of all applicants.

  • Personal Statement - Required of all applicants.

Please provide any ADDITIONAL information about yourself which you feel might be helpful in reviewing your application for admission to the UCLA music major. This statement should be typed or printed on 8 1/2" x 11" paper and should not exceed two pages. ATTACH your statement to the questionnaire. You may submit a copy of the essay included with your UC application or write a new personal statement.

  • Audition Confirmation Form – Required of all students who have an audition appointment. Appointments will be emailed to applicants after December 1 and throughout early January. Pre-screening applicants do not need to fill out this form until they have been notified of selection for the live audition.

  • Pre-Screening CD – Required for Cello, Clarinet, Flute, French Horn, Piano, Saxophone, Trombone, Trumpet, Viola, Violin and Voice applicants.

Applicants planning to audition for Cello, Clarinet, Flute, French Horn, Piano, Saxophone, Trombone, Trumpet, Viola, Violin or Voice are required to submit a pre-screening audition. Compact disc (CD) is the preferred format and may be submitted any time following the submission of your admissions application, but must be received NO LATER than January 8, 2010. Only those applicants selected from the pre-screening evaluation will receive an appointment for a campus audition. Send your Pre-Screening CD with your Questionnaire and Personal Statement.

  • Pre-Screening Portfolio– Required for Composition applicants.

Applicants who indicated Composition as their primary interest must submit a minimum of three written scores, due by January 8, 2010. Following a review of all submitted scores, selected students will receive an appointment for an interview/audition with our composition faculty. Send your Pre-Screening Portfolio with your Questionnaire and Personal Statement.

Please do not mail any supplemental materials prior to the submission of your Univeristy of California admissions application.
