AUDITION CONFIRMATION AND REGISTRATION FORM

The UCLA Department of Music requires an audition as part of your application for admission. You should have received an audition appointment for your major instrument or voice. However, if you are proficient in more than one area, you may request an audition in a second instrument. Confirmation of your appointment for a second instrument will be sent to you if there is sufficient space in the schedule. All applicants must complete both part I and part II of this form.

We recommend that you bring your own piano accompanist to the audition, especially if you are uncomfortable performing with someone with whom you have not practiced. The Department of Music will provide piano accompanists if necessary. If you think you will need one of our accompanists, you must complete part II of this form. A music theory exam is required of all applicants, and will be offered every hour between 10:30 a.m. and 5:30 p.m.
